Hospital at Home enables patients to receive hospital-led care in their own homes instead of staying in a hospital bed. What is hospital at home?

It is a hospital programme where the patient is treated and monitored in their own home instead of on a ward. The hospital stays responsible for the care. Technology like SmartFuture’s provides the devices, the monitoring and the link back to the care team.

How are patients monitored safely at home?

Clinical teams decide which patients are suitable for hospital-at-home care and configure monitoring requirements and alert thresholds. SmartFuture helps the care team monitor readings, identify readings that require review and coordinate follow-up.

What does the patient need at home?

It depends on the care programme. Patients who need connected monitoring can receive a tablet and paired medical devices. Other patients can use their own devices and submit readings through a simple web link.

What if the patient is not good with technology?

The tablet shows one simple screen and stays plugged in. For most patients the only step is taking the reading — the device sends it by itself. Caregivers and nurses can reach the patient by video from the same screen.

Yes. SmartFuture can integrate with EMR/EHR and other hospital systems through supported APIs and integrations. We work with your IT team to align the integration with your workflows, infrastructure and security requirements.

Yes. SmartFuture is white-label, so the platform and the patient app can carry your hospital’s name and look.

Which conditions can it be used for?

It is not limited to one condition. Care plans are configured per patient group, so the same platform supports post-surgical recovery, heart and lung conditions, diabetes, maternity care and general recovery after a hospital stay.
